Our Commitment
No one will be denied access to our services due to inability to pay. We offer a discounted/sliding fee schedule based on family size and income.
The Chautauqua Center Mobile Health Care, based in Jamestown, NY, is part of a Federally Qualified Health Center (FQHC) system. An FQHC is a community-based provider that receives federal funds to provide healthcare in underserved areas. FQHCs must meet a stringent set of requirements, including providing care on a sliding fee scale and operating under a governing board that includes patients.
Since opening our first site in 2013, The Chautauqua Center has been recognized as a health leader in the region. In 2017, we proudly achieved the designation of a Patient Centered Medical Home (PCMH), reflecting an excellent track record of providing advanced primary care by taking the extra step for patients and those in the community. We were further honored in 2021 with the designation of Patient Centered Specialty Practice.
In 2019, we received the Economic Development Award from the Chautauqua County Chamber of Commerce, an honor reserved for an organization that has contributed most to the economic development, quality of life, and stability of our region. That year we served close to 10,000 patients in the Chautauqua County area. We generated over $16m for our local economy and supported 174 local jobs.
In 2021, the Health Resources & Service Administration (HRSA) recognized The Chautauqua Center in three categories of achievement: Access Enhancer, Advancing Health Information Technology (HIT) For Quality, and COVID-19 Data Reporter. The Amercian Heart Association awarded us a silver designation for our work in the TARGET:BP program focusing on community heart health.
