Healthcare providers - TCC Health - The Chautauqua Center

A Federally Qualified Health Center

Federally Qualified Health Centers (FQHC) are community-based health clinics that provide comprehensive primary care services to everyone, regardless of their ability to pay. We offer affordable care on a sliding scale so everyone receives the healthcare they need.

FQHCs receive federal and other government funding to provide primary care services in underserved areas. This support allows TCC Health to offer services on a sliding fee scale based on income.

TCC Health offers a wide range of preventive and primary care services, including:

  • Regular checkups and physicals
  • Immunizations
  • Women’s health services
  • Mental health and substance use counseling
  • Dental care and prevention
  • Management of chronic diseases like diabetes and high blood pressure

Community Health Centers

Community Health Centers (CHC) are non-profit clinics that provide affordable, high-quality healthcare to everyone in their community. They focus on serving people who may face barriers to care, like those with limited incomes, no insurance, need confidential care, or who live in areas with few medical providers.

Although we care for each individual patient, we measure success by improved health of the overall community.

A Patient-Centered Medical Home

A Patient-Centered Medical Home (PCMH) isn’t just a place, it’s a way of providing care. In a PCMH, you have a healthcare team that:

  • Focuses on YOU as a whole person, not just your illnesses.
  • Coordinates your care with specialists when needed.
  • Makes it easy to access care and get your questions answered.
  • Helps you set health goals AND supports you in reaching them.

A PCMH is a model of healthcare designed to give you the best possible experience. Benefits of a PCMH include:

  • Better communication with your doctors and nurses
  • Care that focuses on prevention and wellness
  • Easier access to appointments and shorter wait times
  • Help managing chronic conditions
  • A true partnership with your health care team

Think of your traditional doctor’s office as a collection of individual services. A PCMH is like having a “home base” for your health!
